The City of Lake Worth Beach Florida has established procedures for filing property damage
claims against the city.Any claimant seeking to make a property damage claim against the City of Lake Worth
must submit the Property Damage Claim Form to the City of Lake Worth Beach, Human
Resource Department, 7 N. Dixie Hwy, Lake Worth Beach, FL 33460 under the following
guidelines:
Claim forms must be fully completed and signed. Failure to do so may result in denial of
claim. Indicate Not Applicable (N/A) where appropriate.
All estimates, receipts, and insurance statements must be submitted with the claim form.
Failure to do so may result in denial of claim. Claimants that dispose of property or have
repairs completed prior to submitting claim do so at their own risk. It is highly recommended
that electronically dated photographs accompany submissions.
One repair estimate and copy of any coinciding report filed with relevant city departments
(such as police reports) are required for all property damage claims.
Claims alleging defective city streets, curbs, sidewalks, sewers, trees, or facilities should
be reported to the appropriate department (such as: Buildings, Streets, Recreation, etc.) as
soon as possible. Such reports should include the exact date and time of incident, a detailed
description of the incident and the location (such as street name and address, nearest
intersection, or landmarks (if any).
All insurance payments claimant receives in connection with a City of Lake Worth Beach
claim must be reported in writing to the City Attorney’s office within forty-eight (48) hours of
receipt of insurance payment. All such correspondence should reference claimant’s name and
date of loss. Additionally, claimant must include a letter from the insurance carrier when
carrier denies coverage for property damage as subject to this claim. If the damaged property
is not insured, claimant must so indicate this at bottom of claim form.
Claimants must agree to a visit by a claims adjuster to determine actual monetary loss if
deemed necessary by city representatives. Refusal to permit damage assessment may result in
denial of claim.
Claimant address or telephone number changes must be reported in writing within 48
hours to the City Attorney’s office.
Payment of claim requires claimant signature releasing the City of Lake Worth Beach, FL
from any further liabilities for the same incident.
