Citation Hearing Request

1. You may request a hearing by providing the information requested above. Have it notarized and sign this request form. Please read this form carefully.

2. Parking violation hearings are held before the City of Lake Worth Beach, Special Magistrate. The officer/employee or who issued the citation may represent the City of Lake Worth Beach at the hearing along with any evidence. The Special Magistrate determines whether the alleged parking violation was committed, based upon the testimony and evidence presented at the hearing and may dismiss. If the Special Magistrate finds that a violation occurred they will set citation fees and hearing costs, which may range up to $1,500.00.

3. If the Special Magistrate determines that you (or someone operating your vehicle with your permission) committed the alleged parking violation, you will be deemed to have waived your right to pay the original amount indicated on the parking notice. A citation fee, plus hearing costs may be imposed as stated #2 above. If you are found not in violation, the Special Magistrate may dismiss the citation and waive fees and costs.

4. Once a hearing time is requested, payment cannot be accepted either by the City of Lake Worth Beach Parking Division or on the City of Lake Worth Beach Parking Division website. You must appear before the Special Magistrate.

5. Failure to appear before the Special Magistrate scheduled hearing date may result in you being adjudicated in violation with a citation fee (as applicable) and hearing costs, which may range up to $1,500.00. There are no cancellations/changes regarding days and times.

6. You must provide the City of Lake Worth Beach, Parking Division with your current home address. All notices pertaining to your parking violation and hearing appearances will be sent to the address that you provided on this form.

7. This form may be submitted online, by mail to the City of Lake Worth Beach, Parking Division at 501 Lake Ave. Lake Worth Beach, FL 33460 within fifteen (15) calendar days of violation issuance.

8. Hearings are normally held on the last Thursday of the month at 8:30 am or adjusted as necessary in City Hall Chambers. A letter/notification will be mailed to you, prior to the hearing date providing all details of the hearing. All hearing forms turned in before the close of business the second Friday of the month will be heard that month, all others will be heard the following month.

9. In order to be processed, this form must be received by the City of Lake Worth Beach within fifteen (15) calendar days of issuance of the citation. No appeals will be processed after the 15th day. Please note we are closed on weekends and major holidays.

I, the above named parking citation recipient, acknowledge receipt of the above stated City of Lake Worth Beach, Florida municipal violation and desire to enter my plea of NOT GUILTY and request a hearing with the City of Lake Worth Beach, Florida as stated in this document.

I understand that by electing to go to hearing before a Special Magistrate, I may be subject to additional fines and costs if I am found in violation of the city code as stated in this document.
